A Consideration on New Coronavirus COVID-19 by a Japanese Clinician in Akihabara

What I mean is that the new coronavirus COVID-19 is not a threat in developed countries.
Probably because the mortality rate of COVID-19 is strongly dependent on the environmental factors in the area where it occurred.

I am a general internal medicine specialist working in Akihabara Tokyo, Japan. Many patients consult us every day including common cold.
Since I am neither a virologist nor an infectious disease specialist, this entry is only a personal expectation.

In our country the new coronavirus COVID-19 from Wuhan has been in vogue for several months. Even cluster-type epidemics sometimes occurrs in several different areas.
However, the death toll is only six, and mortality rate is still very low.

Initially, the outbreak in Wuhan had a death rate was over 10% at first.
Recalling SARS, the people of the world were afraid.

However, according to reports the mortality rate has dropped to about 0.17%.Currently the mortality rate even in China is about the same rate and stably low.
Although the total number of patients in Japan can't been estimated, I expect mortality rate is much lower.

What does this upper "fact" mean?

I believe that "mortality rate of COVID-19 is highly dependent on the local public health care system, medical environment and human density".

One of the dangers is that people live closely and share large amounts of virus particle in closed rooms.

It may also be related to not having received a pneumococcal vaccine or the like.
Usual control of diabetes is also important.
The investigations for "COVID19 virus-associated pneumonia" and comorbidities such as DM, hypertension, COPD, etc will reveal new knowledges as well as COVID19 itself.

Japan has been building and maintaining health care insurance system for many years that allows all citizens to receive medical services. No matter where the patient lives we can freely access to domestic medical institutions, we can get advanced medical care from the first step.

In Japan diabetic patients have good treatments under the insurance medical treatment system. Additionaly, most elderly people are raceived pneumococcal vaccine for free in Japan. Maybe the medical system in Wufan was not the same. Thus COVID19 will not be a threat in such a well-equipped area.

I want to insist that the importance of COVID19-related pneumonia and the local public healthcare system including medical equipments. I expect that the mortality rate will be vary and different widely depending on the country's health system respectively.

I expect in the case of this virus, it is impossible to prevent at the shore.

The virus itself may be just a different type of "runny nose comon cold virus" in developed countries.
I wanted to let the world know from Japan that it would be okay not to worry too much.
The mortality rate is too low to measure in Japan so far.　We must avoid stalling the economy with such infections with focusing to the each scientific fact. Don't be scared by reputation and wrong speculation.

日本の臨床医の新型コロナウイルスCOVID19についての一考察

私が言いたいことは、新型コロナウイルスCOVID19は先進国においては脅威にならないということです。
たぶんCOVID19の死亡率は、発生した地域の環境的なものに強く依存していることが推測されるからです。

私は秋葉原で働いている総合内科専門医です。ウイルス学者でも感染症専門医でもないので、記載は私見にすぎません。

日本では、武漢から発生した新型コロナウイルスCOVID19が数か月流行しています。クラスター型の流行も散発しています。しかしながら、死者数はわずか6人にとどまっています。

当初、武漢でアウトブレイクした際には死亡率は１０％を超えていました。SARSを思い起こさせ、世界の人々は恐れていました。

ところが、その後死亡率は０．１７％ほどに低下しました。現在中国全体の死亡率も同じぐらいです。
日本での全患者数が把握できていませんが、死亡率はそれよりもずっと低いことを私は予想しています。

こう言った事実は何を意味しているのでしょうか？

私は、COVID19の死亡率はその地域の生活や医療環境に強く依存しているからだと思っています。

人々が密接に暮らして、密室空間で大量のウイルスを共用することが危険の一つになります。
また、肺炎球菌ワクチンなどを接種していないことも関係するでしょう。
COVID19そのものだけでなく、COVID19ウイルス関連肺炎の検索も進むことでしょう。

あるいは、医療システムが整っていなかったかもしれません。
日本では、国民全員が医療を受けられる保険システムを有しています。
患者さんがどこに住んでいても、国民は医療機関を自由に受診できます。

そして、先進的医療を最初のステップから受けることができます。そういった医療体制の整った地域では、COVID19は脅威にならないでしょう。

COVID19関連肺炎、地域の医療体制や衛生環境、そういったものが重要というお話をいたしました。
先進国では、ウイルスそのものは鼻かぜのウイルスに過ぎないかもしれません。

私は、世界の皆さんに過剰に心配しなくて大丈夫だということを日本から発信したかった。
今でも日本の死亡率は限りなく低いままです。
真実を見つめることで、こんな感染症ごときで経済が失速しないようにしなくっちゃいけないと思います。
噂や憶測におびえることはよくありません。
